Central Intake/Transition Coordinator



PRIMARY DUTIES AND RESPONSIBILITIES:
The Central Intake/Transition Coordinator will facilitate both outreach initiatives and program admissions for all services provided by the Agency with the exception of Early Intervention and Pre-School.  The Central Intake/Transition Coordinator will maintain a global understanding of all Agency services and will effectively communicate with prospective consumers, parents/caregivers, school personnel, community partners, and Agency staff, while assisting with the intake and OPWDD eligibility application processes. The Central Intake/Transition Coordinator will be responsible for developing contracts and billing.
