Community Support

Community Habilitation

Community Habilitation services are provided in the community to assist in developing and enhancing the skills necessary to live in the community as independently as possible.

All individuals participate in developing and implementing their own service plan centered on their strengths, needs, and choices. Common focus areas include:

  • financial management
  • household maintenance
  • health care
  • personal hygiene
  • nutrition
  • community inclusion
  • budgeting
  • safety
  • social skill development

Services are provided by qualified staff in a group or one-on-one setting that takes into account the person’s lifestyle and commitments.

Respite

In many families, caring for an individual with a developmental disability often falls on the parents, siblings, and other family members. Respite offers temporary, short-term care for individuals so caregivers can take a much needed break or see to personal matters. Services are provided by qualified staff in a one-on-one setting in the home, or in a recreation group setting in the community. Respite takes into account the individual  needs of the family and the individual with a developmental disability.

Family Support Services

Family Support Services (FSS) are ancillary services offered to individuals with developmental disabilities that live with their families. Family Support Services offers programs and supports for family caregivers, enhancing family stability and preserving family unity.

Goals include:

  • To support quality of life for an individual and their family.
  • To prevent premature or inappropriate out-of-home placement
  • To reunite and strengthen families
  • To enhance parenting skills
  • To maximize the potential of the family member with a developmental disability

The service also facilitates the Family Reimbursement Program which reimburses expenses for health and safety goods and services. The program enhances family stability by relieving the financial burden of caring for an individual with developmental disabilities.

Individualized Support Services

Individualized Support Services (ISS) assists adults who wish to live independently.  ISS is a housing subsidy calculated based on an individual’s income.  For those eligible for ISS may also provide a transition stipend.
