Step #2: Care Coordination Selection

When we have answered all of your questions about the services offered through our agency, you will select your Care Coordination provider and let OPWDD and the providers know whom you have chosen.  We will help guide you with any additional questions or concerns you may have.

Step #3: Plan Development

With the assistance of your Care Coordinator, you will develop a plan to cover all your needs. The Arc of Yates is here to help meet you and your family's needs.

This year the Arc of Yates received $14,000 to provide guardianship services for people with intellectual and developmental disabilities who would not otherwise have a legal guardian or advocate.
