Step #2: Care Coordination Selection

When we have answered all of your questions about the services offered through our agency, you will select your Care Coordination provider and let OPWDD and the providers know whom you have chosen.  We will help guide you with any additional questions or concerns you may have.

Arc of Yates Receives Council on Quality and Leadership (CQL) Accreditation



The Arc of Yates received formal notification today that they have met the standards required to receive an accreditation from the Council on Quality and Leadership (CQL).  The accreditation comes after a  4-day onsite visit by CQL  reviewers who met with staff, community members, and individuals supported by the Agency.

CQL has been the preeminent organization for more than 40 years in defining, measuring and improving the quality of life for all people, including those with developmental and intellectual disabilities. The Arc of Yates becomes the only human services agency within Yates County to receive the CQL  Quality Assurances Accreditation.

The person-centered philosophy championed by CQL and embraced by the Arc of Yates places the individuals receiving services in control. The programs and services they participate in are not dictated by what others think is best for them, but by the individual. With guidance from the Arc of Yates, individuals with intellectual and developmental disabilities make decisions based on their interests, passions, dreams and desires.

The accreditation process began in 2015 when the Arc of Yates created a CQL implementation team made up of  8 employees representing different departments within the organization. This  team worked with every department and program in the Agency to examine how closely the Arc of Yate’s practices aligned with CQL standards.  The team evaluated    10 Key Factors and  46 Indicators during the process, not only determining if the Arc of Yates met each requirement but documenting and proving each item for CQL reviewers.

“This process was a great opportunity for our Agency to examine every program and every procedure, ensuring that we continue to be person-centered in the work we do,” Arc of Yates President & CEO Daniele Lyman said.  “It was an excellent opportunity to take an in-depth look at the work we do, with the help of the individuals we serve and their families, and see how we can raise the bar even higher to succeed in our goal of providing a Lifetime of Opportunities.”


During the process  reviewers met with several staff members, individuals supported by the Agency, and their families. 


CQL accreditation was awarded as a result of the Arc of Yates demonstrating proficiency in the following areas:

(1) Basic Assurances (ensuring fundamental safeguards related to health, safety and human security);

(2) Personal Outcome Measures® (supporting and empowering people to achieve their goals and dreams);

 (3) Shared Values (ensuring accountabilities related to our interactions with people).


Staff and individuals supported by the Agency gathered at the Arc of Yates headquarters on Friday morning to receive news of the accreditation.
