Step #2: Care Coordination Selection

When we have answered all of your questions about the services offered through our agency, you will select your Care Coordination provider and let OPWDD and the providers know whom you have chosen.  We will help guide you with any additional questions or concerns you may have.

Step #3: Plan Development

With the assistance of your Care Coordinator, you will develop a plan to cover all your needs. The Arc of Yates is here to help meet you and your family's needs.

Wellness Workshop Series

Part I, “Understanding Cerebral Palsy” 


This workshop will provide a clear definition and broad overview of cerebral palsy, develop an understanding and offer first hand experiences from an individual who has cerebral palsy and from parents who care for their children with cerebral palsy.

Workshop will begin at 5:30pm
